COMPLETE INTAKE FORM

Click BOOK NOW to complete our secure intake form. You will be asked to accept our Terms and Conditions and enter your patient details. Please be sure to input your insurance information, including a copy of the front and back of your insurance card or choose the self-pay option. This is required prior to your first appointment.

SCHEDULE 1-ON-1 SESSION

After you complete the patient details, you will be prompted to provide a reason for the visit so that we can match you with a provider. If a provider is available, you will be prompted to schedule a virtual appointment.

ATTEND YOUR TELEHEALTH VIDEO SESSION

After scheduling, you will receive a confirmation email with instructions to register on our secure patient portal. You must complete all required forms at least 24 hours before your appointment for your session to take place. Log into our secure patient portal at your scheduled appointment time.

Will my insurance cover the cost of my PT session?

Yes, this depends on your insurance plan and coverage. It's possible that you may have some financial responsibility that will be due at each session. It is advisable to understand your benefits and coverage before making an appointment.

Can you still see me if I don't have insurance?

Yes, self-pay options are available.

How will I communicate with my Physical Therapist

You will be able to contact your Physical Therapist through the patient portal.

How is my privacy protected?

We are compliant with HIPAA regulations and take privacy seriously.

Are the therapists verified?

Each Physical Therapist is fully licensed to practice in the state a patient resides.

What is the cancellation policy?

You must cancel at least 24 hours in advance, otherwise you will be charged a $25 cancellation fee.

What is the time commitment?

For best results, we recommend completing 2 sessions per week, on average. However, you & your Physical Therapist will develop a program together that will be specific to you and your needs.

What happens after I sign up?

You will receive an email with the date and time of your video call with a Physical Therapist. If you can’t find the email in your inbox, please check the spam or the promotions folder of your email account.

What do I need for my PT Session?

For your first session, you’ll need access to a computer/tablet/phone with video & audio capability. That's it!

Will Physical Therapy hurt?

While it's not uncommon to feel some discomfort during or after your physical therapy session, you should not feel severe or sharp pain.
